The Ministry of Health is informing us that the Application for Issue of Medical Certificate for Reasons of Public Health Is available for all on https://bit.ly/2wctwgh (Ελληνικά) και https://bit.ly/397nJGa (English).
The mentioned certificate is issued to persons that have to remain under self-quarantine for 14 days due to the return from travel abroad or due to contact with a confirmed incident of COVID-19.
It is noted that relevant travel documents (boarding pass, or flight ticket) should be attached to certify the date of arrival in Cyprus.
The signed applications and the necessary verifying documents should be sent by Facsimile to 22771496 or by email to smichael@papd.mof.gov.cy.
