We are expecting forms in English as well as confirmed by the government ministers.
DOCUMENT A relates to the transport of employees and should be provided signed by the employers/supervisors to employees who will be going to work based the work schedule to be implemented by each organization.
The same document can be also used by self-employed workers.
ΒΕΒΑΙΩΣΗ-ΚΥΚΛΟΦΟΡΙΑΣ-ΕΡΓΑΖΟΜΕΝΟΥ-FORM A
DOCUMENT B which relates to the DECLARATION FOR EXCEPTIONAL MOVEMENT BY CITIZENS will be filled in by each civilian individually when he needs to move outside the house for reasons as detailed in the attached application form.
Alternatively, instead of completing the attached form, civilians send an SMS to 8998 without charge in the form in which it is required in the mentioned webpages.
ΔΗΛΩΣΗ-ΚΑΤ-ΕΞΑΙΡΕΣΗ-ΜΕΤΑΚΙΝΗΣΗΣ-ΠΟΛΙΤΩΝ- FORM B
With regards to the SMS service the procedure of texting to 8998 is as follows:
- Insert number as per purpose required (numbers 1-8) – Text is translated in English in RED.

The list of purposes is translated as follows:
- Μετάβαση σε φαρμακείο ή για αιμοδοσία ή επίσκεψη σε γιατρό – Transportation to pharmacy, for donation, doctor visit
- Μετάβαση σε κατάστημα για αγορά ή προμήθεια αγαθών/υπηρεσιών πρώτης ανάγκης – Transport to store for essential items
- Μετάβαση σε τράπεζα, στο μέτρο που δεν είναι δυνατή η ηλεκτρονική συναλλαγή – Transport to bank where e–banking is not possible
- Απόλυτα αναγκαίες επισκέψεις σε κρατικές υπηρεσίες ή υπηρεσίες του ευρύτερου δημόσιου τομέα και της τοπικής αυτοδιοίκησης – Transport for visiting government and other semi–government services or local councils
- Διακίνηση για παροχή βοήθειας σε άτομα που αδυνατούν να αυτοεξυπηρετηθούν ή που οφείλουν να αυτοπροστατευθούν ή βρίσκονται σε αυτοπεριορισμό ή/και σε χώρους υποχρεωτικού περιορισμού (καραντίνα) – Transport to support people in need who need to be self–quarantined or are quarantined
- Μετακίνηση για σωματική άσκηση ή για τις ανάγκες κατοικίδιου ζώου, εφόσον δεν υπερβαίνουν τα δύο άτομα και περιορίζονται σε γειτνιάζουσες με την κατοικία τους περιοχές – For physical exercise or pet needs up to groups of two people.
- Μετάβαση σε τελετή (π.χ. κηδεία, γάμος, βάφτιση) από συγγενείς πρώτου και δεύτερου βαθμού που δεν υπερβαίνουν τον αριθμό των 10 προσώπων – Transport due to ceremonies (e.g. wedding, baptizing, funeral) for 1st or 2nd degree relatives for a number not exceeding 10 people
- Δήλωση οποιουδήποτε άλλου σκοπού μετακίνησης που μπορεί να δικαιολογηθεί με βάση τα μέτρα απαγόρευσης της κυκλοφορίας Any other reasons that can be justified based on the needs to prohibit transport
FOR EACH PERSON IN A VEHICLE A SEPARATE SMS PER INDIVIDUAL OR DECLARATION FOR EXCEPTIONAL MOVEMENT WILL NEED TO BE SHOWN TO THE RELEVANT AUTHORITIES.
